Caregiver’s Authorization Affidavit with Instructions

A caregiver’s affidavit allows a caregiver to enroll a child in school.  If the caregiver is related to the child, this affidavit can be used to consent to medical care on behalf of the child. If the caregiver is not a relative, then this affidavit will only allow the caregiver to consent to school-related medical care.
